We all dislike filling out paperwork, but our office needs these forms in order to properly evaluate you, coordinate your care with your Primary care physician efficiently, and to bill your insurance company appropriately.  You can complete most of this paperwork before coming in for your appointment.  See the documents below and complete at your leisure, then bring in all your completed paperwork for your appointment.  At this time we are not accepting these forms via fax.
