We will always try to schedule your appointments promptly.
Please make sure your primary care physician provides us with a referral before your appointment.
Call within 24 hours to notify us if you can not make your scheduled appointment.
Arrive 15 minutes before your scheduled appointment time in order to properly check in and prepare for your visit.
Co-payment is expected at the time of visit.
We do not refill medications routinely by phone or fax. Please ask for your medication refills at the time of your visit.
We will always work closely with your primary care physician in order to provide the best and most appropriate care for you.
